External rotation, or also known as restitution, is when the baby rotates from a face-down position to facing either one of the mothers inner thighs.

This is the spontaneous realignment of the head with the shoulders.
☾
This movement is necessary for the baby to move its shoulders to fit under the pubic arc.
☾
Restitution begins after the short pause that happens after the baby’s head is born and babies generally do not need manipulation and managing to come earthside.
☾
But we often do not get to see this process unfold naturally when a provider is eager to manipulate this process as soon as the head emerges, which can cause more trauma to the perineum than necessary.

This is clear, visible and beautiful evidence that babies indeed do know how to be born and they are active participants during the process of birth.
