Back in 2003, the WHO recommended removing coached pushing from practice.

Coached pushing is when the birthing person is told to take a deep breath and hold it while someone counts to 10 during at least 3 to 4 pushing efforts per contraction as the parent lies in the supine lithotomy position and a care provider forces the parents legs back against the abdomen. 👈🏻this is how most parents in the U.S. are birthing even though we know it’s not the best and that it creates worse outcomes for parent & baby

The pushing phase is the most stressful time for the baby and it needs careful care to avoid unnecessary interventions.

Why is coached pushing harmful?
♦️Prolonged maternal breath-holding results in maternal apnea and negative maternal hemodynamic changes that lead to decreased blood flow to the placenta and, ultimately, negative consequences for the baby. The baby may develop metabolic acidosis if this type of pattern continues over a long period. These babies are difficult to resuscitate and may not transition well.
♦️Many care providers fail to realize that their aggressive coaching techniques are the cause of nonreassuring fetal heart rate patterns. So now baby doesn’t look good on the monitor and the provider must quickly “get the baby out” 👈🏻often by emergency cesarean from all the aggressive coached pushing. If the baby is not responding well, the best approach is to let the parent & baby rest/recover.
♦️Even parents with an epidural do better with parent led pushing and patience. With epidural anesthesia, pushing can and should be delayed 1-2 hours.
♦️Forcing a parents legs back against their abdomen during pushing results in stretching the perineum and increases the risk of perineal lacerations.
♦️Damage to the parents pelvic floor & bladder increases

When pushing is led by the parents natural instinct, opposed to coaching, there are fewer fetal heart rate decelerations and less of a negative effect on fetal acid-base status and oxygenation.

Parent led pushing is HEALTHIEST, EASIER, AND LEADS TO LESS HARMFUL INTERVENTIONS TO THE MOTHER AND BABY.
